Medical Leave

When you are unable to attend work due to illness or injury, please tell your supervisor right away.

If your illness or injury prevents you from being able to work for an extended period (more than five days), you may need to provide a doctor's note. Please email us and we will work with you to ensure you have the required documentation. 

  • For details about paid medical leave entitlements, as per Collective Agreement, please view your collective agreements.
  • For Medical leave for support staff excluded from bargaining units, review the Sick Leave for Support Staff Excluded from Bargaining Units Policy.
  • Medical Forms: If requested to submit a medical note for your absence, please submit to Employee Health & Wellness' email or by fax to Employee Health and Wellness’s attention at 204.774.2935. We will inform your supervisor on your behalf.
  • Medical notes must include:
    • Start date of leave and end date (or date of reassessment of readiness to return to work)
    • General nature of the illness or injury (specific diagnosis is not required)
    • Must be from a doctor or nurse practitioner
  • The Verification of Sickness Form [PDF] can be completed by your medical provider.
  • The University of Winnipeg is following the recommendations of Shared Health as it pertains to COVID-19. Should you test positive for COVID-19, please review the Province of Manitoba recommendations.

What to Expect — Supervisor

If an employee in your area needs to be away for more than a few days, please reach out to Employee Health & Wellness.

If an employee submits a medical note to Employee Health & Wellness, we will let you know and provide you with the dates that the employee will be away.

If an employee sends their medical notes to you, do not forward or make a copy. Please delete the email and ask the employee to send the medical document to Employee Health & Wellness by email, or fax it to Employee Health and Wellness at 204.774.2935
