When requesting equipment, you must fill out BOTH of the forms in the QR code links.
Requests and appointments for equipment pick-up must be scheduled at least 1
WEEK in advance.
FILMING LOCATION INFORMATION UPDATED NOV 2024
Film students are allowed to use ACTF and main campus buildings for filming with appropriate permission and paperwork. New this year: a longer process for securing 3rd-Party Insurance for non-UW locations, and location releases and personal waivers.
Lead time is critical as new permissions and insurance requirements are in place - plan for at least two weeks for paperwork to be processed.

FILM-SPECIFIC ACTF ROOM BOOKING FORM: A new form has been created specifically for Film Students to use for booking space in the ACTF. Find it here.
UW MAIN CAMPUS LOCATION REQUEST AND AUTHORIZATION FORM: The form for requesting locations around the UW campus (outside the ACTF) has been updated. Several layers of permission are required for filming on Main Campus so a minimum of two weeks notice is required. Please use one form per location. Find it here, or you can pick up hard copies of the form in the department office, 3T03.
INSURANCE: Off-campus location owners may require 3rd-party insurance, which can be obtained through the University. If you are filming in an off-campus location and the owner requires insurance, please REVIEW THE CHECKLIST and COMPLETE THE FORM (a downloadable form in Word). For questions about Insurance contact Melinda in the department office, 3T03, m.tallin@uwinnipeg.ca or 204-786-9955 who will refer you to the appropriate UW person. Detailed information, photos, and contact information may be required before approval is granted so a minimum of two weeks is required.
WAIVERS: Please note: Waivers may be updated on a regular basis; please always check here for the most up-to-date version for each film project!
The forms may be printed and completed by hand, or partially completed on computer and printed, but ALL SIGNATURES MUST BE HAND SIGNED (no digital signatures). Forms can then be scanned and submitted to the ACTF office electronically to m.tallin@uwinnipeg.ca, or dropped off in hard copy during regular office hours.
Personal Waivers: Any non-UW person participating in your film (actor or crew) should complete the appropriate Adult or Child/Youth Waiver.

Location Release waivers are also required for each non-UW location used in your film.

REMEMBER! If you are considering locations off-campus, please make sure you speak to your Instructor about the appropriate authorizations, permits, permissions, insurance and paperwork.
EQUIPMENT DAMAGE REPORTING FOR INTRO FILM and IN-CLASS USE
This report is for damage that is incurred INTERNALLY, in class, only.
This report is not a substitute for a report completed with the Film Equipment Coordinator; it is for when equipment has been taken out in class outside of the Film Equipment Coordinator's purview.
If you have a scheduled time to return the equipment to the Film Equipment Coordinator, please fill in a report with them during that time.
